We customize each group so the cost is always varied. Please complete as much of this questionnaire as you can…the more questions you answer the more accurate your quote will be.
Group Name: Your Name:
Contact Phone: Contact Email:
Address: City: State: Zip: 1). Type of Group/Ages: It is very helpful to know the type of group (friends, business, church group, school, etc.) as well as the approximate ages of the group. Example: Students (ages 14-15) or Sales Team (ages 25-55)
2). Number of people: We break large groups into smaller groups for activities; we call these smaller groups “Activity Groups”. An Activity Group consists of up to 13 people. For example: a group of 24 would take up 2 Activity Groups (12 in each).
3). Number of Activity Blocks: With the exception of our world-famous 900’ Zip Line, all of the activities we offer take up a 1/2 day; an "Activity Block" is a 1/2 day of activity. In one day an Activity Group can do 2 Activity Blocks. For example: Low Ropes course in the morning followed by the Climbing Wall and Zip Line in the afternoon. We can typically fit the Zip Line in with most activities. When figuring out your number of Activity Blocks you do not need to multiply by your number of Activity Groups (i.e. if you are here for 1 full day with 36 people you would have 2 Activity Blocks).
4). Activities: Keeping in mind that each activity takes 1/2 a day, do you have specific activities in mind? Here is a list of activities: Low Ropes/Teambuilding Course, High Ropes/Teambuilding Course, Climbing Wall, Canoeing, Flat-Water Kayak Class, River Kayaking, Mtn. Biking, Caving, Rock Climbing, Outdoor Education, Guided Hiking, Camping, Natural Water Slide, and Tubing. River Kayaking is the only activity that requires a full day (2 Activity Blocks), it also requires the Flat Water Kayak Class be completed as well. By listing activities here you are not bound by your choices at this time, this just gives us a starting point.
5). Number of meals: We have a full service kitchen and just need to know the number of breakfasts, lunches, and dinners your group will be having. For example: If your group is here for just a day you may need only 1 lunch, whereas if you are here overnight you may need 1 dinner, 1 breakfast, and 1 lunch.
Breakfasts Lunches Dinners
6). Number of nights: If you are going to be spending the night we just need the number of nights.
7). Type of Accommodations: If you have a preference of either our Main Lodge (sleeps up to 45) or Sly Lodge (sleeps up to 25) please indicate below. You may also opt for tents.
8). Dates: If you have specific dates in mind or just a time of year it is helpful. It is also very helpful to know if it would be weekdays or weekends. If you know approximate arrival and departure times throw them in as well.
10). Goals/Objectives for your group: Teambuilding, just to have fun, get to know each other better, etc.
11). How well does your group know each other:
12). Miscellaneous: Is there anything else we should know or that you’d like to request?
13). How did you learn/hear about us?: Specifics are greatly appreciated (such as keywords, ads, etc.)
We know you most likely can't answer all of these but the more you can answer the better a quote we can get you. Feel free to email or call with any questions.
